Offering a Blue Line 457 Plan to FOP Member Employees

ICMA-RC will provide you with the following materials:

  1. Employer Implementation Package which includes:
    • Employer Implementation Data Form
    • Resolution
    • EZLink Access Form
    • Blue Line 457 Deferred Compensation Plan and Trust Document
  2. Three copies of the Administrative Services Agreement signed by the Lodge and ICMA-RC.

Please complete the following steps:

  1. Complete the Employer Implementation Data Form and provide authorized signature.
  2. Complete the EZLink Access Form
  3. Sign the Lodge, Employer and ICMA-RC copy of the Administrative Services Agreement. Keep the copy marked "Employer Copy" for your records.
  4. Complete the Suggested Resolution and provide an authorized signature.
  5. Assemble Employer Materials for return to your Lodge
    • _____ Employer Implementation Form
    • _____ EZLink Access Form
    • _____ Employer Resolution
    • _____ Lodge Copy and ICMA-RC Copy of Administrative Services Agreement. Please make sure that an authorized representative signs each copy of the agreement prior to returning it to the Lodge.
  6. Mail Employer Materials to your Lodge.

Keep the following items for your records:

  1. Copy of Employer Implementation Data Form
  2. Copy of Suggested Resolution
  3. Copy of Employer Administrative Services Agreement.
  4. Plan and Trust Document
